Childbirth Preparation Checklist Excel Template — Hospital Bag, Postpartum, and Birth Admin

A free Excel checklist for childbirth preparation: hospital bag, family contacts, postpartum warning signs, newborn checks, birth registration, and Korean Happy Birth applications.

Preparing for childbirth is more than packing a hospital bag. Around the due date, families need to coordinate hospital travel, contacts, postpartum recovery, newborn checks, birth registration, and support applications. This template helps you track childbirth preparation in one shared Excel workbook.

This workbook is not medical advice. If you have bleeding, suspected fluid leakage, decreased fetal movement, severe headache, vision changes, trouble breathing, chest pain, or thoughts of harming yourself or the baby, contact your hospital or emergency services immediately.

  • One-sided leg pain, swelling, redness, or warmth
  • C-section wound concerns
  • Severe anxiety, self-harm or harm-to-baby thoughts
  • Newborn poor feeding, fever, breathing trouble, or worsening jaundice

🏛️ Korean Birth Admin Tasks

The admin sheet covers birth registration, Government24 Happy Birth applications, mother-newborn health management service, local birth grants, leave paperwork, and health insurance checks.

How to Use

Step 1: Fill the Overview Sheet

Enter due date, birth hospital, clinician, and support person. Changing the due date automatically updates task due dates in the checklist.

Step 2: Update Checklist Status

Use the Checklist sheet to assign owners and update status. Completed items feed into the dashboard progress rate.

Step 3: Complete Hospital Bag and Contacts

Hospitals and postpartum care centers provide different items, so adjust the hospital bag list based on your admission guide. Fill real phone numbers in the contact sheet so your support person can act quickly.

Step 4: Track Post-Birth Admin

Use the Admin sheet to track birth registration, Happy Birth applications, public health center services, workplace leave, and insurance tasks. Eligibility and deadlines vary by location and household status.

Tips

Keep One Shared Source of Truth

Avoid multiple copied files. Upload the workbook to Google Drive or keep one cloud copy so everyone works from the same source.

Adjust the Bag List to Your Hospital

The workbook includes a practical baseline list. Delete or add items according to your hospital or postpartum care center guide.

Review Warning Signs With the Support Person

The person who gave birth may be too tired or overwhelmed to judge symptoms alone. Review the postpartum warning sheet with your support person before delivery.
