How to Use
Step 1: Enter Workplace Information
Fill in the company name, industry, location, assessment date, and assessor name.
Step 2: Select Industry Tab
Choose Manufacturing, Construction, or Service to auto-load representative hazards. Modify or add/remove rows as needed.
Step 3: Assess Risk Levels
Select likelihood and severity for each hazard. Risk levels are auto-calculated. Enter improvement actions for items with risk level 3 or above.
Step 4: Review Results and Print
